We have a very customized OneStepCheckout for our shop Rockflame.com. The cart lines are directly placed to checkout page, most of the design is customized and we have also gotten it integrated with Klarna, which is a huge payment provider in scandinavia. I was a little hesitant with purchasing the Enterprise edition, but they have been very helpful and I think I have already gotten my moneys worth!
Also note their paid service is also fantastic! So far everything i have asked for they have been able to deliver. The only unfortunate thing is there is a delay between time zones, but that is more my problem than theirs...hehe. They always respond within a day, even with the time zone issue.

Great extension, makes the checkout system so much easier, and works seamlessly with SagePay Form to redirect. Had a slight issue with some scripting but One Step Checkout support was first class and solved the problems. Although its not cheap it is a highly recommended checkout extension! We're still in development for our store, but I'd like to comment on the fabulous support from the Onestepcheckout team. They're always quick to respond to any query, and even provided a free upgrade to an add-on feature that became available in a subsequent version. Among the extensions that we have installed in our shop, One Step Checkout is really on the top. It is so fast, that it actually has happened that a customer placed an order when they were expecting a confirmation page!
The support is splendid, friendly and extremely knowledgeable. We also payed OSC once for solving a problem with a third party extension. OSC solved it in two hours, though the other company said it would take days.Posted 11/14/2011 Developer Support Functions as described Usefulness -

We've been using this extension for nearly two years I believe. I'm an annoying customer and constantly ask questions about updates and various features. Sometimes this extension has issues with Magento upgrades, but we can't fault the extension creator for this.
The point of this review is to give a big thumbs up for the quality of customer service from this company. They are very quick to respond even though they are many time zones ahead of us. They have definitely given customer support outside of the typical 1 year period and then 6 month period after upgrades. Kind of a 'we provide customer service no matter what, no questions asked.' There is probably only a few other companies that create extensions for Magento that provide that kind of service.
I watch customers very closely through chat software tracking and successful checkouts have definitely increased with the use of this extension. You could get by with the normal Magento checkout, but this checkout has paid for itself very quickly.
Also, I've checked out the other one step checkout extensions on Magento Connect. I believe the reason they are cheaper is because 1) they ripped off the code from this extension and most of them have the old original version of this extension and not thew newer version that upgrades easier and 2) they don't provide support and have the bad reviews to substantiate that claim.
